Job Summary

Provides administrative assistance to the Social Services Director in the areas of resident admissions, discharges, insurance and other administrative processes.  Supports Casey’s Pond directors in other areas as requested.

Essential Functions

  • Assist the Social Services Director in the planning, developing, organizing, implementing, evaluating, and performing administrative procedure in the social services department.
  • Aid with preparation for admission and discharges, including admission/discharge paperwork and other document gathering.
  • Prepare documents for any home health, hospice or outpatient therapy services electronically, document, and send to correct entity prior to discharge and document that in the medical record.
  • Coordinate out of network insurance process ensuring necessary documents are submitted timely and appropriate parties are communicated with effectively. 
  • Assist in the preparation of documents for Medicaid application/renewal with the Professional Medical Information Page (PMIP) and other medical records electronically, add to tracking system and send via secure email.
  • Create a system to manage and track the Post Eligibility Treatment of Income (PETI) needs on behalf of residents and document in the medical record. Assist in submission and tracking of entire PETI process.
  • Assist in chart audits, the Pre-Admission Screening and Resident Review Program (PASRR),  Uniform Long Term Care Assessments (ULTC-100) and Trauma Informed Care program. 
  • Assist with other compliance projects as needed.
  • Coordinate Zoom conferences for Medicaid or Mental Health assessments and assist residents during the call.
  • In collaboration with clinical staff, coordinate the updates and posting of documents including managing move in documents online.
  • Enter documentation into resident chart as needed.
  • Collect We Care Connect information coordinate call following discharge.
  • Create a system to coordinate and schedule care plan conferences with residents, family members and required staff. 
  • Assist residents with various tasks as requested.   
  • Create electronic systems to track Medicaid pending residents, ULTC-100s, PASRRs, audits and other processes as requested.
  • Assist Social Services Director with preparation for various committee meetings.
  • Aid with the coordination of ancillary services with Unit Secretary.
  • Create and maintain as social services process binder.
  • Perform all duties in line with HIPAA compliance.
  • Utilize sound judgment and experience to solve moderately complex problems in adherence with Casey’s Pond Associate Handbook and Behavioral Code of Ethics.
  • Perform other duties as assigned.

Education and Experience  

High school diploma or equivalent preferred; 1 year experience working with seniors preferred.  PointClickCare or similar electronic medical record system experience strongly preferred.

Makes standard and routine decisions based on detailed guidelines with little independent judgment. Problems are generally solved with clear, detailed guidelines or by reporting them to a supervisor.

Knowledge and Skills

  • Experience with PC's and full proficiency with Microsoft Office Software suite.
  • Ability to effectively communicate with proper English grammar and understand and follow written and oral instructions.
  • Ability to read and interpret documents and write/type routine correspondence.
  • Ability to deal tactfully with personnel, residents, family members, visitors, and the general public. Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning. 
  • Ability to follow community policies.
  • Ability to maintain resident confidentiality and adhere to HIPAA policies.

Compensation: 

$21.00/hr - $24.00/hr depending upon experience.
