Job Summary

With a focus on long term stays, the Liaison assists the Director of Nursing (DON) in various functions including assessment of resident care needs and implementing appropriate actions. Serve as backup Infection Preventionist.  Act as lead of psychotropic, wound management and other clinical quality indicators for long term stay residents.

Essential Duties

  • Assists in encouraging, participating and integrating Eden Alternative initiatives by supporting successful aging and person-centered programs and culture. 
  • Ensures that all employees possess and maintain proper assessment and clinical skills to deliver high quality resident care.
  • Supports strategies to improve nursing performance in clinical practice.
  • Sets long term stay goals.
  • Participates in all aspects of Case Management, including care conferences, order reviews, discharge planning, provider relations and resident satisfaction.
  • Coordinates long term stay admission and discharges.
  • Serves as the Hospice champion for the Doak Walker House.
  • Conducts audits, chart reviews and manages incidents.
  • Responsible for oversight of wound management, quality indicators and UDAs.
  • Attends GO meetings and manages GO Carts for long term stay residents.
  • Reviews all lab results for Multi-drug resistant Organisms (MDRO) and consults with IP
  • Responsible for cart audits on the Creekside neighborhood.
  • Communicates with Pharmacy for long term care resident medication needs.
  • Performs medication reconciliations for long term stays.
  • Assists with long term stay grievances and service recovery.
  • Participates in daily clinical stand-up meetings,
  • Assists to ensure effective communication both internally and externally to foster continuity of care.
  • In conjunction with other members of the clinical leadership team, sets the expectation for continuous survey readiness.
  • Utilize sound judgment and experience to solve moderately complex problems in adherence with Casey’s Pond Associate Handbook and Behavioral Code of Ethics.
  • Perform other duties as assigned in line with HIPAA compliance.
  • Other duties as assigned.
  • Provides disease management and education to residents.

Education and Experience   

Education as required to obtain state nursing license (LPN or RN) and a minimum of 3 years relevant experience.  Must have working knowledge of electronic medical records/resident charting systems.

Licenses/Certifications

  • CO LPN/RN
  • CPR certification required
  • Possess Infection Prevention certification or obtain within 3 months

Compensation: 

 $38.46 per hour - $47.11 per hour dependent upon applicable experience
